TERMS AND CONDITIONS

Micro Mold Plastics USA, Inc. requires its suppliers to:

- Notify the organization of nonconforming product; obtain organization approval for nonconforming product disposition, 

- Notify the organization of changes in product and/or process, changes of suppliers, changes of manufacturing facility location and, where required, obtain organization approval, and flow down to the supply chain the applicable requirements including customer requirements. 

-All special processes required by this PO must be performed by qualified personnel. 

-The vendor shall maintain the proper identification and revision status of specifications, drawings, process requirements, inspection/verification instructions and other relevant technical data. 

-The vendor is required to supply product that has a remaining shelf life of at least 80% of original life as of the date of shipment. 

-The vendor is required to retain all records associated with the purchase order for 7 (seven) years or as required by contract. 

-All vendors providing Calibration Services must be certified ISO17025 (or equivalent). All Calibration Certificates must identify standards used and must be traceable to NIST (National Institute of Standards Technology). 

-Certification of material and/or process performed must accompany materials/parts received. 

-COUNTERFEIT PART PREVENTION: 

THE SELLER SHALL TAKE NECESSARY ACTION/S TO ASSURE PRODUCTS ASSOCIATED WITH THIS PROCUREMENT ARE PROCURED DIRECT FROM THE ORIGINAL MANUFACTURER OR MANUFACTURER AUTHORIZED DISTRIBUTORS. THE SELLER SHALL HAVE THE FULL RESPONSIBILITY OF SUPPLYING ARTICLES OF PROPER ORIGIN AND VOID OF ARTICLE/PART COUNTERFEITING OR SUBSTITUTION. THE SELLER SHALL NOT SUPPLY RE-USED, RE-WORKED, MISHANDLED, IMPROPERLY STORED, IMPROPERLY TESTED OR ARTICLES OF UNKNOWN OR UN-VERIFIED PEDIGREE. 

Right of access may be required by Micro Mold Plastics USA, Inc., their customer and regulatory authorities to the applicable areas of all facilities, at any level of the supply chain, involved in the order and to all applicable records. 

-The organization must implement and maintain a Quality Management System. 

-The organization must ensure that persons are aware of: 

-their contribution to product or service conformity. 

-their contribution to product safety. 

-their importance of ethical behavior.
